The Insurance Waiver is required by each player that attends our events. Save yourself time by downloading the file and completing it before the event. This will speed up the check-in process for you and us.

Also if you have any medical conditions then we should know about during the event then download and complete the medical form as well. This informs us of your medical condition, what medication you might be taking and your family contact details in case we need to contact them in an emergency.
